Creation of a Standard EDRN

An standard Medical Certificate of Death is created when you want to register a death to allow families the ability to obtain a burial permit and make funeral arrangements and you have all the required information.

Step 1: Click the death icon, below is the screen you will see.

It is easiest to search for a decedent by using their Saskatchewan health card number, or their name/date of birth by clicking on search now and entering the information. Once this is done, all of their demographic information will be prepopulated.

Once you click search, the decedent’s name will appear and you will then click select. Once selected, you will see the decedent’s name added to the top of the screen.

Step 2: Select standard.  As you have searched by the decedent’s name/date of birth, or health card, the information regarding whether or not the identity is known will be prepopulated for you, as will their name, date of birth, sex, province of residence, and health card number.

 

You will be required to enter the date of death, the location of death and the funeral home chosen by the family if known then click continue.

Step 3: Complete the information related to the the cause of death.

Complete the following information, then click continue.

Step 4: Certifier Type. Choose your role from the drop down menu (attending physician, physician attending after death, coroner, prescribed practitioner [ie: Nurse Practitioner], prescribed practitioner attending after death             [ie: Nurse Practitioner]. Then click certify case.
