1 Patient Encounters Tracking Sheet
Students are responsible for logging patient encounters. Students use a program provided application to track patient encounters. This information is stored in a secure environment by the program. Audits are performed regularly by the Clinical Faculty.
The Patient Encounters Tracking Sheet includes the following information:
- Course number
- Date of Patient Visit
- Time with the Patient
- Patient initials
- Patient age
- Patient gender
- Type of visit (Initial or follow up)
- Primary diagnosis
- Level of preceptor support required (Complete dependence, Developing competence, Independent)
- Purpose of the visit
- Comment section (voluntary) to include brief information if needed
NOTE: The preceptor validates completion of the required practice education hours by signing the preceptor feedback form.
