Practice Education Patient Contact Log and Completion of Hours
Practice Education Hours Defined
- Time spent with patients and consulting the preceptor or other professionals entered on the patient logs.
- Clinical hours are those hours spent at the clinical site engaged in the clinical care of patients within the clinical setting.
- Breaks, meals, and time spent engaged in other activities not related to the care of patients may not be included
- Students completing more than 7.5 hours in a single clinical day are required to take a 30-minute meal break. Meal breaks do not count toward clinical time and must be accounted in the patient contact log.
Students are responsible for logging patient encounters. Students use a program provided application to track patient encounters. This information is stored in a secure environment by the program. Audits are performed regularly by the Clinical Faculty.
The Practice Education Patient Contact Log includes the following information:
- Course number
- Date of Patient Visit
- Time with the Patient
- Patient initials
- Patient age
- Patient gender
- Type of visit (Initial or follow up)
- Primary diagnosis
- Level of preceptor support required (Complete dependence, Developing competence, Independent)
- Purpose of the visit
- Comment section (voluntary) to include brief information if needed
The preceptor validates completion of the required practice education hours by signing the preceptor feedback form.
